Obatoclax甲磺酸盐(GX015-070)是一种化学合成的小分子抑制剂,选择性靶向包括Bcl-2,Bcl-xl, Mcl-1, Bcl-w,A1和Bcl-b在内的Bcl-2家族蛋白(Ki值范围为1-7 μM)[1]。Obatoclax占据Bcl-2蛋白中BH3结合沟槽内的一个“疏水结构域”,抑制这类蛋白活性并诱导凋亡,这个过程是Bax和Bak依赖的。Obatoclax高效干扰在体外分离的具有完整线粒体外膜的线粒体中的Mcl-1/Bax蛋白的直接相互作用,也能抑制细胞内Mcl-1/Bax两个蛋白间的相互作用。这一特性使其克服Bcl-2/Bcl-xl/ Bcl-w-选择性拮抗剂ABT-737以及蛋白酶体抑制剂硼替佐米(bortezomib)对表达Mcl-1细胞没有作用的缺点[2]。通过时间和剂量依赖性的方式,Obatoclax直接诱导体外培养的急性骨髓性白血病(AML)细胞和病人来源的原代细胞发生凋亡 [3],并对实体瘤和骨髓瘤细胞构建的小鼠移植瘤模型表现出抗肿瘤活性[4]。Obatoclax,以单一疗法的方式,正处于晚期恶性血液病患者的临床I期以及骨髓纤维化患者的临床II期研究。另外,Obatoclax与拓扑替康(topotecan)联合治疗,在复发性小细胞肺癌患者的II期临床研究中疗效显著。
