a2-抗纤溶酶是一个分子量为70KD的单链抑制物，其和纤溶酶可迅速形成纤溶酶/a2-抗纤溶酶复合物。其在肝脏中合成，血液中近似含量为1uM（70ug/ml），当血液凝固时20%被交联。PAP的形成分2个步骤，首先，纤溶酶的Lys结合点与a2-抗纤溶酶的羧基端形成可逆复合物，其次，抑制物的结合肽段去除从而形成不可逆的复合物。
PAP在血栓治疗过程中被消耗。PAP复合物的测定结果可提供关于纤溶程度和可能出现出血现象的情况。伴随纤维蛋白形成增加和纤溶增加的疾病，PAP复合物含量也增加。所以，对于许多疾病，纤维蛋白降解产物的水平和PAP的水平呈现相关。这一关系表明PAP的测定是血栓与出血事件的良好指标。
