| 描述 | Milnacipran hydrochloride (Savella) is a serotonin and norepinephrine reuptake inhibitor used to treat major depressive disorders. Milnacipran hydrochloride is an enantiomer of milnacipran, which is used to treat fibromyalgia. Milnacipran hydrochloride and milnacipran have been associated with a low rate of transient elevations in serum aminotransferase levels during treatment and with rare instances of clinically apparent acute liver injury with jaundice. |
| 体外活性 | 在大鼠前额皮质中,Milnacipran（10和30 mg/kg,p.o.）能增加细胞外NA和5-HT的水平,且呈剂量相关性.在抑郁症动物行为模型中,Milnacipran（30和60 mg/kg,p.o.）可使大鼠在强迫性游泳实验中的静止时间显著缩短.在焦虑症动物行为模型中,Milnacipran（30和60 mg/kg,p.o.）也能使大鼠在条件性恐惧压力测试实验中的不动时间明显减短.在自由活动豚鼠下丘脑中,用 Milnacipran（10 mg/kg和40 mg/kg）处理,可使NA代谢产物MHPG分别减少57%和47%.Milnacipran（<40 mg/kg,i.p.）可以剂量依赖地增加自由活动豚鼠下丘脑中细胞外的NA和5-HT的水平. |
| 体内活性 | 超过80%的Milnacipran以本体和葡萄糖醛酸苷的形式经尿液排出,仅有不到10%经CYP3A4酶催化的N端脱乙基作用代谢。在高浓度时,Milnacipran能抑制某某些配体离子通道受体,包括5-HT3A（IC50：185 μM）,NMDA（IC50：58.4 μM）和nACh（IC50：14.3 μM）受体。 |
