【背景知识】 乳糜泻 (CD) 被定义为一种慢性小肠、免疫介导的肠病,由遗传易感个体暴露于膳食麸质(存在于小麦、大麦和黑麦等谷物中)引起。它是一种常见的自身免疫性疾病,影响世界许多地区约 1% 的人口2-3。 CD的遗传易感性主要与几个HLA系统基因有关。尤其是基因型 DQ2 和 DQ8,它们存在于 95-98% 的乳糜泻患者中,并且出现在一般 4-5 的 20-30% 中。这种非 IgE 介导的食物过敏会导致大量吸收障碍,其特征是绒毛完全萎缩和上肠隐窝增生。 CD 的血清学检测随着时间的推移而发展。最初这些测试涉及抗麦醇溶蛋白抗体 (AGA) 的检测,但由于缺乏敏感性和特异性,随后发现其诊断价值较低3。因此,血清学研究已经发展到检测针对自身抗原(例如组织转谷氨酰胺酶 (tTG) IgA / IgG)的抗体,这仍然是确定 CD 的关键检测方法。 已经证明,乳糜泻患者会合成抗醇溶蛋白的特异性抗体,称为脱酰胺醇溶蛋白肽 (DGP)。抗 DGP 抗体是识别麸质不耐受受试者的高度特异性标志物,与抗麦醇溶蛋白抗体不同,后者也存在于健康受试者或受其他肠道疾病影响的受试者中,因此特异性较低 6。因此,新一代的测定涉及检测这些抗 DGP 抗体。已发现这些测定对抗 TG 测试的表现相对较好,并且已发现检测抗 DGP IgG 的测定在 IgA 缺乏症患者中非常准确。 在儿科人群中,有一些迹象表明抗 DGP 检测可以提供进一步的用途,因为这些检测在高风险和低风险人群中都具有非常高的诊断准确性7-8。几项调查评估儿科人群(2 岁以下儿童)中 DGP 抗体水平的研究表明,即使在已确定 TG 水平正常的情况下,该标志物 9-10 的诊断准确性也为 100%。 观察到抗 DGP IgA 和 IgG 与诊断年龄呈负相关,7 岁及以下儿童的值显着高于老年患者 11。此外,已发现在一些具有 CD11 遗传风险的儿童中,抗 DGP 抗体先于 TG 抗体出现。这产生了使用抗 DGP IgA/IgG 检测作为某些婴儿群体中 CD 的早期指标的可能性。对婴儿的进一步研究发现,DGP 抗体水平与肠道损伤的严重程度相关12。还建议在所有受试者中使用抗 DGP IgA 和 IgG 抗体测试,无论年龄大小,表现出提示 CD 的症状,并且 t-TG 或 EMA 自身抗体不存在或仅以低滴度存在 13。 无麸质饮食的乳糜泻患者显示抗 t-TG 和抗麦醇溶蛋白抗体逐渐减少。 IgG 抗体滴度比 IgA 抗体水平下降得更慢。
