腮腺手术 腮腺手术包括腮腺混合瘤及腮腺恶性肿瘤切除术。涎腺肿瘤中混合瘤占80%-90%,其中以腮腺混合瘤为最常见。由于腮腺混合瘤潜伏着恶性变的危险,治疗的原则应是早期摘除。单纯地将肿瘤从包膜或腺体中摘除,则术后复发率可高达20%-45%。其原因被认为是腮腺混合瘤的包膜是由腮腺组织压缩而成,并非纤维性真性包膜,故包膜本身就可能含有肿瘤细胞,而且这个包膜往往非常薄或者极不完整,常有肿瘤组织自包膜突出,侵入正常腮腺组织内。因此,手术时应将肿瘤连同包膜及肿瘤周围的正常腮腺组织
[应用解剖] 腮腺位于耳垂下前方,前界在下颌支、嚼肌面上,后界为乳突,上为外耳道,下为下颌角下方。 腮腺分为深浅两叶,中间有峡部连接。面神经穿行两叶之间。腮腺外面有来自颈固有筋膜的致密被膜,并伸入腺体内,分腺体为许多小叶。 面神经自茎乳孔穿出后,从腮腺后上方进入腮腺,在深入约0.5~1cm处分为面颞支和面颈支两大支,前者再分为颞支、颧支、颊支,后者再分为下颌缘支、颈支,最后终止于面部肌肉[图1]。 图1-1 面神经与腮腺深、浅叶的关系 图
慢性腮腺炎为最常见的涎腺炎症,本病以前称为慢性化脓性腮腺炎,现在分为复发性和阻塞性两类。 慢性复发性腮腺炎,以儿童多见,发病年龄多在5岁左右,男性发病率高,为周期性发作,年龄越小,间歇时间较短,易复发,随着年龄增长,发作次数减少且症状减轻,青春期后一般逐渐自愈。半数以上患者首次发病有流行性腮腺炎接触史。临床主要表现为腮腺反复肿胀不适,有脓性分泌物自导管口溢出。 慢性阻塞性腮腺炎又称腮腺管炎,大多数为局部原因所致,中年人发病率高,多为单侧受累,临床主要表现为阻塞症状和腮腺反复
