摘要

目的:为识别nivolumab敏感性的预测因子,本研究对参与REVOLUTION试验的转移性肾细胞癌(mRCC)患者外周血自然杀伤细胞(NKs)和调节性T细胞(Treg)进行了评估。实验设计:对57例接受nivolumab作为至少二线治疗的mRCC患者及62例健康供者进行了外周NKs与Tregs的表型及功能纵向评估(0-1-3-6-12个月)。采用多变量逻辑回归确定独立预测因子,使用0.632+内部交叉验证避免过拟合。基于3个月临床疗效的最佳截断值应用于无进展生存期(PFS)和总生存期(OS)分析,并绘制PFS与OS的Kaplan-Meier曲线。结果:治疗前,mRCC患者呈现高频率的NKp46+NKs、NKp30+NKs、KIR2DL1+NKs、KIR2DL2/DL3+NKs及PD1+NKs,且NK细胞脱颗粒功能降低;同时Tregs、PD1+Tregs、Helios+Tregs和ENTPD1+Tregs频率升高。经3个月治疗达到临床缓解的应答者,在治疗前表现出显著低CD3+、高KIR2DL2/DL3+NKs、高PD1+Tregs和高Helios+Tregs特征。多变量分析显示,仅KIR2DL2/DL3+NKs和Helios+Tregs是nivolumab疗效的独立预测因子。KIR2DL2/DL3+NKs >35.3%的患者OS更长,而Helios+Tregs >34.3%的患者PFS显著延长。nivolumab治疗1个月后,应答者呈现低CD3+、高NKs、高KIR2DL2/DL3+NKs及高ICOS+Tregs特征,其中CD3+和KIR2DL2/DL3+NKs为nivolumab疗效的独立预测因子:低CD3+(≤71%)与较长PFS显著相关,而高KIR2DL2/DL3+NKs(>23.3%)与PFS及OS均相关。结论:治疗前Helios+Tregs/KIR2DL2/DL3+NKs评估及治疗后1个月CD3+/KIR2DL2/DL3+NKs检测可预测mRCC患者对nivolumab的治疗反应。
