感染西尼罗病毒可引起包括脑炎在内的一系列临床症状,西尼罗病毒(WNV)是一种潜在的嗜神经病毒,它可引起人群中无症状的感染或者是发热。人或动物的感染最初在西半球并无证实,直到1999年在纽约首府爆发了该疾病,自此以后,该疾病扩散至美国的其他地方,到2003年,该疾病在46个国家发生,并且引起超过9800人发病,大多数西尼罗感染者无任何症状,小部份人可能发展为包括发热﹑头痛﹑身体疼痛﹑皮肤发疹﹑淋巴结肿大在内的轻微的临床症状,在临床上,人西尼罗病毒感染引起的发热是具有自限性的急性发热性疾病,伴头痛﹑肌肉酸痛﹑多发性关节痛﹑发疹﹑胃肠道症状和淋巴结炎。少于1%的感染者发展成严重的疾病,包括脑炎和脑膜炎。

试剂盒可用于定性检测人血清中的西尼罗病毒IgM抗体。
本实验用于临床实验室诊断临床上有与脑膜脑炎症状相一致的病人西尼罗病毒的感染。阳性的结果必需用蚀斑减少中和试验(PRNT)来证实,或者使用现有的CDC的的指南来确诊该疾病。
警告:一些西尼罗病毒IgM实验的测试样品中可能含有肠道病毒的抗体,应注意其交叉反应。
【检测原理】
试剂盒是基于双夹心法原理。未知的样本与质控品加入到微孔(包被有抗人IgM)中进行温育,样本中的特异性抗体会结合到微孔板上。洗板之后分别加入西尼罗河病毒抗原(WNRA)和普通细胞抗原(NCA)进行反应。再次洗板后加入酶联物,用于检测所结合的抗原。第三次洗板后加入底物液,一段时间后加入终止液终止颜色反应,溶液变为黄色。450nm下测量OD值。结果中WNRA与NCA的比值ISR可用于结果的判定。

然后将扩增后的CAR T细胞注入到患者体内。注入之后,如果一切顺利按计划进行,T细胞会在患者体内繁殖,经设计的受体会引导T细胞识别并杀死那些具有相应表面抗原的癌细胞。

没有选择情况下的一种可能选项80%以上被诊断为ALL的儿童患者起源于B细胞,这是儿童ALL的主要类型,能通过高强度化疗治愈。

该研究的作者报道,在27位完全反应的患者中有19位仍处于缓解期,其中15位患者没有接受进一步治疗,4位患者退出试验并接受其他治疗。
根据POB试验(包含ALL儿童患者)的最新数据显示,20位患者中有14位出现完全反应。该研究的首席研究员以及POB的首席研究员医学博士Daniel W. Lee报道称,12位患者在经过CAR T细胞治疗后骨髓中没有白血病细胞(被称为早期造血细胞),其中有10位继续接受干细胞移植并保持无癌生存。
Lee博士说:“我们的研究结果明确表明,如果患者对化疗不再有反应,CAR T细胞疗法可作为骨髓移植的有益的过渡治疗。”
在MSKCC和NCI进行的成年患者I期临床试验中也发现了类似的结果。
2014年2月发表的调查结果显示,MSKCC试验的16个参与者中有14个出现完全反应,有些情况下是在开始治疗后2周内发生的。在这些符合要求的患者中,由7位接受了干细胞移植,现在仍保持无癌生存。
NCI引导的CAR T细胞试验中有15位成人患者,其中大部分患有晚期弥漫性大B细胞淋巴瘤。医学博士James Kochenderfer及其NCI的同事们报告称,试验中的大部分患者出现完全或部分反应。
The amplified CAR T cells are then injected into the patient. After the injection, if all goes well as planned, the T cells will multiply in the patient and the designed receptor will direct the T cells to recognize and kill those cancer cells that have the corresponding surface antigen.
Although this method has so far been limited to small-scale clinical trials, the use of these designed immune cells to treat patients with advanced cancer has produced some significant results.
This process is based on a similar form of ACT advocated by patients with advanced melanoma by Steven Rosenberg, MD, and colleagues from NCI Surgery.
“The CAR T cells are more effective than any other immunotherapy being studied,” said Crystal Mackall, MD, a pediatric oncology (POB) doctor at NCI. Even so, researchers in the field caution that there are still many aspects of CAR T cell therapy to be studied. However, the results from the early experiments have been quite optimistic.
Dr. Rosenberg wrote in the article Nature Review: Clinical Oncology that CAR T cell therapy may eventually "become the standard therapy for certain B-cell malignancies," such as ALL and chronic lymphocytic leukemia.
A possible option in the absence of a choice More than 80% of children diagnosed with ALL originate from B cells, which is the predominant type of childhood ALL and can be cured by high-intensity chemotherapy.
Stephan Grupp, MD, PhD, of the Children's Hospital of Philadelphia (CHOP), said that for those cancer patients who have relapsed after high-intensity chemotherapy or hematopoietic stem cell transplantation, the remaining treatment options can be said to be “almost nowhere”. Dr. Stephan Grupp is the head of research leading the pediatric CAR T cell assay. He pointed out that this kind of therapy is a new choice that these patients urgently need.
In the CAR T cell assay for adults and children with leukemia and lymphoma, the patient receives a designed T cell that targets the CD19 antigen, which is present in almost all B cells (including normal cells and cancer cells).
According to a survey published on October 16 in the New England Journal of Medicine, in the CHOP trial, in cooperation with researchers from the University of Pennsylvania, 27 of the 30 patients had their cancer signs disappear (complete response).
The authors of the study reported that 19 of the 27 fully responding patients were still in remission, of whom 15 did not receive further treatment and 4 patients withdrew from the trial and received other treatments.
According to the latest data from the POB trial (including children with ALL), 14 of the 20 patients had a complete response. The study's lead researcher and chief investigator of POB, Daniel W. Lee, reported that 12 patients had no leukemia cells (called early hematopoietic cells) in the bone marrow after CAR T cell therapy, and 10 of them continued to receive stem cells. Transplant and maintain cancer-free survival.
Dr. Lee said: “Our results clearly show that if patients no longer respond to chemotherapy, CAR T cell therapy can be a useful transitional therapy for bone marrow transplantation.”
Similar results were also found in the Phase 1 clinical trials of adult patients performed by MSKCC and NCI.
A survey published in February 2014 showed that 14 of the 16 participants in the MSKCC trial had a complete response, and in some cases occurred within 2 weeks of starting treatment. Of these eligible patients, seven received stem cell transplantation and they still remain cancer-free.
There are 15 adult patients in the NCI-guided CAR T cell test, most of whom have advanced diffuse large B-cell lymphoma. James Kochenderfer, MD, and his NCI colleagues report that most or all of the patients in the trial experienced a complete or partial response.
