CD40-CD40L通路是连接先天性与适应性免疫的核心共刺激轴，但其临床开发长期受制于系统性给药的剂量限制性毒性。2025–2026年，这一困局出现了方向性松动：肿瘤领域不再执着于全身性CD40激动剂，而是转向FAP×CD40双特异性DARPin/抗体与瘤内给药，在实现肿瘤微环境重塑的同时将全身毒性降至最低；自身免疫领域则凭借Fc沉默/Fc工程化的第二代CD40L抑制剂，接连在SLE与干燥综合征的III期试验中取得阳性结果。机制层面，CMTM6维持B细胞膜CD40稳定性、以及CD40双表位抗体
